Responsibilities
Serves as the legal Pharmacist-in-Charge and coordinates pharmacy services at assigned site. Provides pharmacy services to our customers. Reviews medication orders issued by authorized prescribers for accuracy and appropriateness. Reviews and interprets patient data bases for medication therapy related problems. Develops a therapeutic plan and communicates the plan to appropriate provider and/or health care team member. Works with pharmacy technicians to provide accurate and timely distribution of medications which may include critical, high expense medications, chemotherapy, controlled substances and hazardous pharmaceutical substances. Is a drug information resource to patients, medical staff, nursing staff, and ancillary department personnel. Encouraged to be a Pharmacy resident preceptor and a student mentor for pharmacy students.
Qualifications
- Required: Graduate of an accredited College of Pharmacy. Doctor of Pharmacy (Pharm.D.) degree, preferred completion of an accredited pharmacy residency, and a minimum of one-year relevant pharmacy practice experience or equivalent experience. Equivalent experience includes a minimum of two years relevant pharmacy practice experience.
- Preferred: Proven history of managing a large, progressive pharmacy operation. Formal Training: or experience in quality improvement, financial analysis, patient safety, The Joint Commission and other accreditation standards, expense management and team building. Strong commitment to excellence. Project management and coordination skills. Computer skills including word processing, spreadsheet and data base applications. Ability to communicate effectively, works collaboratively with others, and works independently with minimal supervision.
License or Certification
- License to practice Pharmacy in the state of Minnesota required
