Case Manager - DD Anoka County (Part-Time)

LOCATION: West St. Paul working with Anoka County countracts

HOURS: This position starts as part-time 20 flexible hours/week and will go to full-time as cases come in.

JOB SUMMARY: We are currently seeking a contracted case manager to be responsible for conducting assessments, creating and reviewing care plans, coordinating services with the client and interdisciplinary team, connecting clients with community resources and ensuring waiver coordination and documents meet lead agency guidelines.


* Licensed social worker preferred or 4 year degree psychology, sociology, or closely related field from an accredited college/university

* At least one year experience working with individuals with developmental disabilities

* Experience with Rule 185 and DD waiver preferred

* Knowledge of MA, Medicare and MN Healthcare Programs strongly preferred

* Background clearance

* Ability to effectively communicate in English, written and verbal

* Driver's license, acceptable driving record and proof of auto insurance required

* Must be at least 18 years of age
