Case Aide Resource Counselor

Prevention and Aftercare Program is a Los Angeles County Department of Children & Family Services (DCFS) funded program designed to invigorate and restore the development of healthy behaviors for each family.
The Help Group, through its many specialized clinical and educational programs, provides assistance to children and families within their communities through its Child and Family Center therapeutic and educational programs. The Help Group has provided services through the Department of Children and Family Services and Department of Mental Health (DMH) since 1985
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide case management services for high need, at-risk families, including the identification and pursuit of resources necessary and appropriate to meet client needs.
- Secure adequate and appropriate arrangements for housing, vocation, education, mental health, and other social service or public assistance programs.
- Collaborate with IHOCs to create an individualized Family Support Plan, ensuring strategies, interventions, and services are family-centered, flexible and community based.
- Participate in team meetings, which include multi-disciplinary professionals, the child and family, extended family members, and informal community supports.
- Assist in obtaining outcome measures and other documents as assigned and complete all required documentation, reports and outcome evaluation forms in accordance with programs guidelines
- Participate in the development of an appropriate Crisis/Safety Plan and Individualized Service Plan when needed
- Follow up with exited or transitioned families monthly for a period of 6 months top ascertain successful connection to requested resources/linkages, provide additional resources if needed and maintain a log of all follow up contacts.
WHAT YOU WILL NEED:
- BA or BS in social work, psychology, counseling or related field with a minimum one (1) year experience working with at risk children, adolescents and families.
- Will consider AA degree in related field with minimum three (3) years of experience working with at risk families and and with diverse population
- Familiarity with principles of family-centered, strengths-based service and group facilitation.
- Ability to function as part of a multi-disciplinary team.
- Knowledge of local community and its various resources
- Strong ability to maintain resource library, expanding it regularly and keeping team members updated on the expanding aides available to families
- Ability to collaborate with community partners, other agencies and resources, including P&A's subcontractors
- Bilingual in Spanish strongly preferred.

Salary Range:$21.08 Hourly
