Catholic Charities, Diocese of Trenton is a leading provider of Behavioral Health services in Mercer and Burlington Counties. We are currently seeking Hourly (19 hour / week) LPC, LCSW, LMFT and or RN to provide comprehensive rehabilitation needs assessments as well as develop individualized rehabilitation plans in partnership with consumers in our dynamic Community Support Services Program in Mercer County.

Competitive Rate: $45 - $60 hr (contingent upon experience)

Key Responsibilities:

• Conducts Comprehensive Needs Assessments as well as develop Individualized Rehabilitation Service Plans.

• Completes clinical documentation in accordance with Community Support Services Regulations agency standards, accreditation and funding source requirements.

Job Requirements:

Must possess current NJ License (LPC, LCSW, LMFT, RN).

Must possess knowledge and familiarity with evidence based practices

Must have at least 2 years’ experience working in community based behavioral healthcare setting

Must be computer literate as well as proficient with using electronic health records
