Licensed Practical Nurse, Long Term Care, Per Diem

POSITION SUMMARY: Under the direct supervision of the Registered Nurse, the Licensed Practical Nurse will coordinate and supervise the implementation of the nursing care plan for the assigned Resident Nursing Unit. Such supervision must be in accordance with current federal, state, and local standards, guidelines and regulations that govern our facility. He/she, will collaborate with other leaders and staff in the development, management and operation of the assigned Nursing Unit, and will operate under the department philosophy, standards and policies. Ensures that the highest degree of quality care is maintained at all times, under the direction of the RN. The LPN contributes to the resident assessment and care plan under the direction of the RN. # EDUCATION/CERTIFICATION Must have an LPN license in the State of Connecticut and be a graduate of an approved by the State of Connecticut, Licensed practical Nurse or Licensed Visiting Nurse program.# EXPERIENCE One year of nursing experience in a geriatric population is preferred.# COMPETENCIES Must demonstrate effective English verbal communication and interpersonal skills.# ESSENTIAL DUTIES and RESPONSIBILITES: Disclaimer: Job descriptions are not intended, nor should they be construed to be, exhaustive lists of all responsibilities, skills, efforts or working conditions associated with the job.# They are intended to be accurate reflections of the principal duties and responsibilities of this position.# These responsibilities and competencies listed below may change from time to time. # # ######### Job-Specific Competency 1.##### Collects accurate and thorough physical, cultural, psychosocial assessment, and documents in the medical records. 2.##### Consistently discusses information obtained with RN to gain a comprehensive view of resident#s status and needs including but not limited to all changes in resident#s condition. 3.##### Demonstrates and understanding of psycho/physiological processes and applies this knowledge to detect changes in residents status, and notifies nurse supervisor policy. 4.##### Identifies learning needs of assigned residents and families, identifies appropriate resources to assist in the educational process and accurately completes patient/education tool. 5.##### Accurately completes necessary documentation on an ongoing basis in a timely manner per department policy. 6.##### Notifies resident#s family and responsible parties of any changes in condition, keeping them informed of the resident#s status. 7.##### Collaborates with the interdisciplinary team and participates in meetings with the family/responsible party regarding the status of resident and plan of care. 8.##### Implements prescribed medical, nursing, psychosocial interventions as appropriate by applying clinical expertise appropriately in a variety of settings. 9.##### Notifies MD of changes in resident#s condition under the direction of a RN and documents communication accurately. 10.# Consistently assists the RN in individualizing the plan of care and completes nursing summaries prior to RCC. 11.# Identifies changing discharge needs and keeps the Nurse Manager well informed. 12.# Appropriately delegates duties and tasks to team members and directs and monitors delivery of care by CNA#s. 13.# Consistently prepares and administers medications and IV fluids accurately, according to department policies. #
