Community Health Navigator

* Purpose of Position

The navigator provides care coordination assistance to help under-served women access health services related to screening, diagnostics, and treatment for breast, cervical, and colon cancer. Successful navigators can proactively conduct outreach, collaborate with community organizations and medical providers, and provide culturally responsive and trauma informed approach to care.

Essential Duties and Responsibilities

* Developing partnerships with culturally diverse organizations, and conducting targeted outreach to women over the age of 45 or those at high risk for cancer.

* Providing group and one-to-one education on prevention and screening, related to breast, cervical, and colon cancer.

* Arranging screening, diagnostics, and treatment appointments with medical providers on behalf of the client.

* Accompanying client to medical clinics, hospitals, behavioral health providers, and social service agencies as necessary.

* Communicating information to clients in a culturally responsive way that is easily understandable and emotionally supportive via in-person, email, phone, and text. Explaining complex health forms and materials in a way that helps clients feel at ease, empowered, and supported.

* Communicating information to health providers, partner organizations, and other staff in a professional way that demonstrates clear thinking, health knowledge, respect, professionalism, and clear understanding of roles and boundaries.

* Providing warm referrals and direct access to translators/ interpreters, health and food benefits, transportation services, affordable housing/ shelter, domestic violence shelter, immigration & naturalization services, employment training & services, mental health and chemical dependency treatment, and other services.

* Entering information in a secure online system in a HIPAA-compliant manner, with signed release of information consent form.

* Attend staff meetings and training as directed.

* Provide monthly reports to management including time sheets, outreach and education reports, and expense & mileage reimbursement reports.

* For clients who are low income and have high deductible health insurance, the navigator will help clients apply for financial assistance through the Breast, Cervical, and Colon Health Program.

Position Requirements, Competencies and Qualifications

* Satisfactorily pass criminal history and reference checks;

* Proficient in MS Office Suite, e.g. Excel, Word, Outlook, etc.

* Valid WA driver's license and automobile insurance in good standing;

* Reliable vehicle to make appointments and transport clients;

* Respect for all cultures, and ability to interact with diverse groups;

* Excellent oral, written, and interpersonal communication skills;

* Strong project management skills and ability to manage competing deadlines;

* Ability to maintain effective relationships with staff, volunteers, community and national partners;

* Ability to work independently; and

* Strong organizational, analytical, and problem-solving skills.

3.1 Educational Level.

* Must have high school diploma, GED or foreign equivalent.

* Bachelor's degree in any field preferred or combination of some college and community health work experience.

3.2 Experience in Related Field.

2-3 years as care coordinator, community health worker, navigator, counselor, or related role.

3.3 Unique Expertise/Certification/Registrations

The following are desired but not required: Community Health Worker training, cancer prevention, chronic disease management, Motivational Interviewing training, or Mental Health First Aid training.

* Environmental parameters:

* Ability to work in an office environment

* Physical demands:

* Type: sedentary work ability to perform administrative duties, typing, filing, bending over to low files, reaching high files

* Ability to exert up to 20 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move an object

* Work involves sitting most of the time, but may involve walking, standing, twisting, reaching, bending and driving for extended periods of time.

Work Schedule

* Ability to work up to 20 hour week and on call when necessary

VISUAL

* Near Vision:

* Clarity of vision at 20 inches or less.

* Mid-range Vision:

* Clarity of vision at distance of more than 20 inches and less than 20 feet.

* Far-range Vision:

* Clarity of vision at 20 feet or more.
