Responsibilities

Position Summary:

A Facilitator is responsible and accountable for the coordination and facilitation of outcomes management for Nursing Directors and entities of St. Joseph's Medical Center. The Facilitator roles are: COMMUNICATOR: Keeps St. Joseph's informed of results, progress and future plans for Quality. COORDINATOR: Supports operations in improving outcomes, links entities to promote improved system outcomes, assists with information needed by teams. PROMOTER: Presents, publishes and coordinates events to promote Quality and employee involvement. TEACHER: Expert instructor in QAPI and statistical tools. COACH: Serves as a resource and coach to employees, Management, Nursing Directors, Senior Management, Councils, Physicians and Teams. Accountable for integration of QAPI, Clinical Pathways, Report Cards, Clinical Outcome Studies, Satisfaction Studies, Redesign Outcomes and implementation of all aspects of Quality Improvement, including TJC Standards compliance.

Qualifications

Minimum:

* Bachelor's Degree in Clinical Field

* Current California RN License National Certification

* Five years' experience in clinical field

* Two years' experience in QAPI

* Computer skills to include WordPerfect, Excel, MIDAS+ Meditech

* Knowledge of TJC, Federal and State Licensing requirements

* Statistical analysis skills (i.e, statistical process control)

* Clinical knowledge

* Team dynamic skills, including conflict resolution
