Stamford Hospital is seeking a Fulltime Pharmacist to join the Team The Pharmacist will report to the Director of Pharmacy, this position is responsible for providing quality pharmaceutical care for the patients of The Stamford Hospital.

* Accurately prepares and dispenses medication orders per physician request according to established policies, procedures and protocols.

* Provides timely and accurate drug information to other health professionals and lay persons when required. Participates in counseling sessions for patients, educating on side effects, indications and directions for use.

* Assures appropriateness of therapy and dose for all patient populations including age-specific care.

* Monitors patient therapy in accordance with departmental policies and procedures, protocols, and programs.

* Carefully identifies, prevents, intervenes, and follows up on any drug related problems, Pharmaceutical Care Quality Improvement programs.

* Supervises pharmacy support personnel based on current laws and regulations.

* Completes required continuous training and education requirements, including state skills necessary to provide quality pharmaceutical care to all patients.

* Ensures safe and appropriate cost effective medication therapies for patients according to established policies, procedures and protocols.

* regulated and departmental specific requirements; and demonstrates knowledge and including any actual or potential adverse drug events by participation in Departmental

* Assists with pharmacokinetic consult services and drug dosing per organizational protocol

* Assures that therapeutic substitution policies are followed based on individual patient need.

* Participates in the quality improvement and medication use review activities of the department.

Required Experience

B.S. Pharmacy, Pharm.D. preferred.

License or eligible for license as a Registered Pharmacist in Connecticut.

Previous experience in an institutional setting preferred.
