Job Description

The Resource Navigator is responsible for providing clerical support for the Care Management team, assisting the team with problem-solving and facilitating inpatient and outpatient care. This position proactively addresses potential or actual transportation and lodging needs as identified by the interdisciplinary team. The Resource Navigator assists patients and families with navigating and accessing external resources, support systems, and information by telephone or in person. This position assists families to navigate internal resources by supporting the telephone triage process.

Job Requirements

Understanding or working knowledge of Care Management models and principles preferred.

Demonstrated competency in Microsoft Office Suite required. Ability to empathize and communicate with those persons facing economic, social, health, age,and educational barriers.

Ability to proactively identify, communicate and advocate for identified patient / family needs within the framework of an interdisciplinary team.

Demonstrated interpersonal and professional skills, including but not limited to professional appearance, tact, punctuality, planning, dependability and the ability to work effectively with a wide range of individuals in a diverse community. Strong communication skills-both oral and written.

Ability to navigate cultural differences to meet patient / family needs.

Negotiation skills; manages differences of opinion / conflict constructively.

Excellent customer service and service recovery skills.

Willingness to understand different perspectives and points of view; understands and respects roles and responsibilities of others.
