In this position you will:
- Provide leadership and continuity of care guidance for patients with chronic diseases and/or complex disease states. Use knowledge of illness, diagnoses and treatments in comprehensive, holistic assessments of patients.
- Provide teaching to patients/families related to patient’s diagnosis, pathology, medical and nursing treatment plans, discharge needs and health goals; documents each element of care per organizational and unit standards.
- Utilize appropriate resources to gather data and assess patient care needs; make ongoing assessments of the patient including but not limited to: physio-biological, cognitive/emotional, spiritual and social, managing life with chronic disease.
- Identify possible problems that could lead to an emergency/crisis situation and takes appropriate action to de-escalate the potential for the situation to occur.
- Maintain complete documentation of all critical and essential aspects of medical history and information in the patient’s file. Manage patient related activities in an outpatient, ambulatory clinic and/or inpatient, acute care setting.
- Assist in designing and implementing planned care interventions through a combination of evidence-based guideline/pathway utilization and individualized care planning.
- Mobilize resources and manages the system in response to patient need according to an interdisciplinary plan of care.
Required qualifications for this position include:
- Graduation from an approved, accredited school of nursing
- Currently licensed as a Registered Nurse (RN) in the State of Washington
- 2 years of work experience in an RN role
- Work experience revolving around medications and IV solutions in terms of purpose, administration, side effects and contraindications and administers according to established standards and provider orders
- Experience managing complex patients in an ambulatory setting
Preferred qualifications for this position include:
- Bachelor's Degree in Nursing
