Responsibilities

The Manager of Social Work provides leadership and supervision to the social work team. This role also designs policies, procedures and processes that support the psychosocial-related discharge planning needs of the patients served. The Manager is responsible for providing programmatic support for complex patients who present in the Emergency Department, Geropsychiatric unit, acute care and the Intensive Outpatient Care Program. In addition, the Manager of Social Work partners with the Manager of Care Transitions and the interprofessional team to ensure policies and procedures are created that support the psychosocial needs as patients transition to the appropriate next level of care.

The Manager of Social Work is responsible for maintaining the highest level of knowledge and understanding of the role each Social Worker plays in the care and discharge planning of our complex patients. The manager is expected to assess for and address identified gaps in knowledge, training, performance, advocacy, and any other skills required to ensure that patients are provided with the highest level of service possible.

Qualifications

Education Background

* Master of Social Work is required.

Licensure, Registration, Certification

* Holds a current Washington State License.

Special Skills/Training

* Holds a proven ability to function in a complex environment, with continually shifting priorities.

* Is able to organize and present data in a clear, concise, and easily understood manner.

* Able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient being served.

Work Experience

* Requires a minimum of 5 years Social Work experience in a health care setting, preferably in an acute care hospital.

* Requires experience in Care Management or discharge planning.

* Requires experience in change management.

* Requires a minimum of 2 years of supervisory or managerial experience.
