Job Description Summary

Successfully promote and support the mission of Catholic Charities of Northeast Kansas, as guided by the Catholic Social Teachings, while serving as primary contact for initial administration of patient referral and admission calls to the Catholic Community Hospice program. Coordinates all aspects of the patient intake process including establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and coordinating the insurance verification and authorization processes. Ensures accuracy and confidentiality of all clinical records, in compliance with all regulatory requirements.

Essential Job Functions/Responsibilities

* Serves as the primary contact for initial administration of patient referral calls and communications regarding patient admission to the Catholic Community Hospice program.

* Completes intake for new clients.

* Coordinates the admissions process from the initial request for admission to the final authorization ensuring all aspects of patient confidentiality are maintained at all times.

* Ensures all intake data for admissions requests are collected, completed and communicated according to the established standard operating procedures and in compliance with all regulatory requirements.

* Obtains the necessary information (e.g., demographic, financial, clinical, and other pertinent data) from referral agency to initiate the process.

* Interviews patient, patient's representative, or referral source to obtain the necessary personal and financial data to determine eligibility for admission.

* Utilizes the appropriate call tracking and Brightree system applications to ensure accurate documentation of calls.

* Promotes the efficiency of the admissions process by answering phones and assisting callers in a professional and timely manner escalating issues to supervisor as necessary.

* Works closely with applicable billing groups to request and obtain insurance verification in a timely manner.

* Works with RN's and LPN's on staffing schedules for Vacations, Call-In's or additional needs of case mangers during the day.

* Works with Case Managers on Angel Vigil Needs. Staffs the Angel Vigil with internal staff and/or Agency.

* Collaborates with the Director of Community Relations and Program Development, Catholic Community Hospice staff, facilities, the physician, and hospital staff to ensure an efficient and effective admissions process and to quickly and efficiently resolve issues when they arise.

* Ensures a seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care and coordination of care with multiple service providers.

Position Qualifications

* Prefer at least three (3) years of healthcare sales and admissions experience. LPN or bachelor's degree preferred.

* A minimum of one year experience with clinical systems and time reporting approval processes
