This is an RN Admissions Nurse position for ARLINGTON HEIGHTS. 10pm - 10:30am. This RN will be required to work every Other Saturday or Sunday. NO 'on call.'

The RN Admissions Nurse is a resource registered nurse to patients, family members and staff of hospitals and facilities. They are responsible for sustaining and increasing referral sources from hospitals, facilities and other community sources. The Registered Nurse - Admissions uses their knowledge in pain and symptom management and the end-of-life in order to educate these referral sources about hospice. The Registered Nurse - Admissions is responsible for working with physicians to discern hospice eligibility and complete an initial assessment for those patients who meet criteria.


* Admits patients to hospice services

* Conducts initial meetings with patients and families to inform them about hospice services and obtains consent for care

* Performs follow up meetings and phone calls from information visits

* Sustains and increases referral sources with hospitals, nursing homes and other community sources

* Documents visits and phone calls with patients, families and referral sources

* Conducts discharge planning of hospital patients that are being transferred home to be admitted onto care with Hospice of Northeastern Illinois

* Mentors and trains new registered nurses

Experience and Skills:

* Associates degree in nursing required

* Registered nurse licensed in the state of Illinois required

* Valid driver's license, auto insurance, and reliable transportation required

* Certified hospice and palliative nurse preferred

* Two (2) to five (5) years of experience as a registered nurse in healthcare, preferably hospice

* Hospice admissions, geriatrics and oncology experience preferred

* Strong verbal, written, interpersonal, and organizational skills required in an interdisciplinary team-oriented environment

* Strong conflict resolution, multi-tasking skills, creative thinking, and problem-solving skills are critical to success in this role

* Knowledge of Medicare, Medicaid and insurance beneftis a plus

* Proficiency in working with Cerner or Allscripts, or other electronic medical record (EMR) system, and/or database experience preferred
