Eliot's Behavioral Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that are evidence based, provide rehabilitation and hope to those in services. Eliot's Behavioral Health Community Partner (BHCP) program is an innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) redesign within Massachusetts.

Eliot is seeking a Care Coordinator to provide primary coordination of service delivery specific to the needs and preferences to its BHCP enrollees. The Care Coordinator will work with a multidisciplinary team to provide person-centered Comprehensive Assessments and Treatment Planning. In this role, you will develop relationships with enrollees, their PCP and Care Team. Eliot is seeking a highly motivated and organized individual to manage a caseload of 80 enrollees. Provide independent care management and active connection to all services and community supports available to each enrollee.


* Promote hope and empowerment through meetings and support with individuals.

* Empower individuals to exercise autonomy in all aspects of their life. Assist enrollees with identifying choices and developing skills in self advocacy.

* Provide side by side interventions to enrollees that focus on problem solving, skills training, modeling behavior, optimism and encouragement.

* Assist the team and enrollees in providing on-going assessment of symptoms and responses to Care Team interventions.

* Participate in the development of Assessments and Treatment Plans for each engaged enrollee.

* Ensure that treatment goals are culturally competent, strength based, realistic, attainable and promote recovery focused on an enrollee's goals for wellness.

* Engage each enrollee in their treatment planning process and their preferences are embraced, collaborated with the enrollee's PCP, and shared with the enrollee's Care Team.

* Maintain all documentation related to assessments, treatment plans, enrollee contacts, collateral contacts and any other documentation necessary.

Schedule: M-F, 9a - 5p

Rate: $50k annually


Must have bachelor's degree or master's degree in psychology, social work or related field and three years' experience. Or must be a Certified Peer Specialist or Certified Recovery Coach. May substitute one year of experience with a college degree in the human services field if a Peer Specialist or Recovery Coach and must be either receiving services or received services in the past from the mental health system and have self-knowledge to manage their mental illness and be well along in their recovery. Must have a strong commitment to mental health recovery, substance abuse disorders, care coordination, and wellness. Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses, individuals with substance abuse disorders, and those with complex medical conditions. Must have ability to communicate clearly and effectively with individuals and engage individuals in assessment, person centered planning, and treatment. Must have ability to provide flexible services, high quality customer service, and develop partnerships internally and externally. Computer literacy and skill in the use of computers and related software applications. Ability to consistently meet all necessary background and credentialing requirements. Must have valid drivers license and reliable transportation. Must have reliable telephone notification system for emergency contact.
