Responsibilities

The Clinical Pharmacist provides pharmaceutical care as required by the Medical Center's patients. The Clinical pharmacist performs clinical and educational functions required in drug monitoring, with emphasis on appropriateness, safety and cost-effective drug therapy. The Clinical pharmacist is actively involved in quality improvement activities of medication use throughout the medical center as assigned. This position also functions as a staff pharmacist up to 50% of the time as assigned. He or she applies CHW Core Values to the performance of every job duty. Qualifications

Two year's experience in a hospital pharmacy and/or completion of an ASHP-accredited residency program.

1 year's experience with demonstrated competency in area of specialty 1-2 years of experience with a minimum of 1 year in area of related field

Complete annual safety training.

Complete and maintain department specific training.

PC Computer skills preferred Board Certified specialty by ACCP or equivalent Board Certified

Current registration in California as a pharmacist.
