& Major Responsibilities Evaluates, plans, and implements treatment programs to ensure maximum patient outcomes. Involves patient and family in goal setting and completes timely documentation. Consults with referring practitioner as needed and remains current on treatment techniques.
Maintains knowledge of, and complies with, all relevant laws, regulations and policies, procedures and standards.
Actively participates in creating and implementing improvements to achieve clinical, satisfaction and/or efficiency outcomes.
Evaluates and develops a treatment plan that is individualized to the patient and reflect the evaluation findings and are prioritized based on input from patient, family and team.
Performs treatment procedures that adhere to standards of practice of the American Physical Therapy Association.
Consults with referring practitioner to ensure quality care and treatments are provided to patients.
Completes daily progress notes that reflect patient s response to treatment, actual performance, and an assessment as to status, relevant to the treatment plan and eventual discharge needs and treatment summaries if needed as evidenced by chart review.
Supervises physical therapy students, physical therapy assistants, and physical therapy aides.
Provides educational materials and methods appropriate to the patient and includes family/caregiver in education and training as needed.
Applies knowledge of therapeutic techniques during the evaluation and throughout the course of treatment and adjusts the treatment plan regularly and appropriately as patient status changes as evidenced by the treatment plan.
Addresses discharge planning needs from initial evaluation to discharge as evidenced by team feedback and documentation. Qualifications: Education:
Bachelor's degree in Physical Therapy required. Doctorate degree preferred.
No experience required. 1 to 3 years of previous experience preferred.
