JOB DESCRIPTION

BACKGROUND

Job Title: Youth Coordinated Entry Case Manager

Reports To: Associate Director of RHY East

FLSA Status: Non-Exempt

PURPOSE OF JOB

To maintain the Youth Coordinated Entry system in St. Louis City and County. To provide case management and advocacy services for runaway, homeless and at-risk youth.

This position is Tuesday - Saturday 11am-7pm.

ESSENTIAL RESPONSIBILITY NO. 1 - 80 % OF TIME

Description: Maintain the Coordinated Entry System

Task No. 1 Respond to Coordinated Entry helpline calls for youth (ages 16-24) experiencing a housing crisis in St. Louis County and St. Louis City, within 24 hours.

Task No. 2 Conduct the TAY-VI-SPDAT to determine eligibility and housing need, as well as Prevention and Diversion screening.

Task No. 3 Provide referrals to internal and external community resources to assist youth with systems navigation, basic needs, and other resources and supports as needed.

Task No. 4 Enter data into HMIS system, and adhere to all HMIS guidelines.

Task No. 5 Respond to phone referrals, enter data into the youth housing waitlist, and collaborate services with St. Patrick's Center.

Task No. 6 Provide housing navigation to youth on waitlist whenever possible to meet their needs.

Task No. 7 Partner with SOP team to provide housing navigation and direct street outreach activities.

ESSENTIAL RESPONSIBILITY NO. 2 - 10 % OF TIME

Description: Develop and maintain relationships with partner agencies

Task No. 1 Engage in relationships with service providers and community organizations in order to reduce barriers for youth accessing services.

Task No. 2 Network with community resources to establish new partnership opportunities to build on existing partnerships for SOP.

Task No. 3 Attend community and service provider meetings/events to increase collaboration.

Task No. 4 Participate in activities related to Coordinated Entry in St. Louis County and St. Louis City.

ADDITIONAL RESPONSIBILITIES - 10 % OF TIME

Task No. 1 Support SOP team by assisting with street-level outreach as needed.

Task No. 2 Represent YIN at community outreach events.

Task No. 3 Participate in after-hours on-call rotation to support youth in crisis.

Task No. 4 Attend all relevant agency meetings and trainings.

Task No. 5 Other duties as assigned.

FORMAL EDUCATION/CREDENTIALS (IF ANY) NORMALLY ASSOCIATED WITH JOB

Necessary: Bachelor's Degree (or equivalent) in Social Work, Psychology, Sociology or related field.

KNOWLEDGE

Familiar with at-risk youth behaviors and barriers

Working with hard to reach or resistant individuals and families

Knowledge of how to access community resources

Understanding of appropriate boundaries with clients

EXPERIENCE

Working with at-risk youth

Collaboration with other professionals

SKILLS

Crisis intervention, needs assessment, and ability to provide referrals. Must be able to develop positive rapport with hard to reach youth. Must be skilled in building and maintaining relationships with external service providers. Excellent oral and written communication skills. Networking abilities. Creativity and flexibility in serving RHY population. High degree of work ethic and honesty. Computer and data entry skills and organization.

ENVIRONMENTAL CONDITIONS

Time spent in the office completing phone calls, needs assessments, and data entry. Time spent at community organizations for case management with youth or attending meetings.

PHYSICAL REQUIREMENTS

Ability to drive a van to transport youth. Ability to fully supervise groups of youth. Ability to lift 20 lbs. to carry supplies or donations.

DRIVING REQUIREMENTS

Required Eligibility Classification

Must obtain class E license

Must be insurable under YIN's auto liability coverage

OTHER REQUIREMENTS

Ability to work effectively with a diverse team.

Individual must be sensitive to the socioeconomic and cultural characteristics of the service population.
