Job Details


* Responsible for providing direct psychosocial counseling and support to patients and families in keeping with the spiritual and cultural frame of reference for each individual.

* Responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet patients' and families' psychosocial needs.

* Demonstrates the ability to perform comprehensive initial psychosocial and financial assessments and effectively collaborates with the IDT and patients and families for a holistic Plan of Care.

* Prepares social histories to establish service needs, taking into consideration: a) family relationships, b) the meaning of illness to the patients and families, c) the cultural attitudes in relationship to illness, d) the meaning of the home situation to patients and families, e) the financial impact of illness on the family.

* Stays well informed about available community resources and utilizes them effectively in meeting patient and family needs.

* Creates a plan for initial and on-going education of patient, family and providers in areas of palliative care/end of life care, Relationship Centered Care, and care of the whole person: body-mind-spirit.


* Masters in Social Work required

* Licensed in State of Michigan as a MSW.

* Current CPR Certification.

* Minimum of one year of clinical work experience, preferably in the health care field. Experience with dying patients and their families is helpful.

* Proficient in use of electronic medical records.

* Demonstrates an ability to establish a high level of rapport with patients/families, placing them at ease across a range of cultures, ethnicities, spiritual, environmental, physical and emotional backgrounds.

* Demonstrated ability in individual, family, group counseling.
