Job Description

Job Summary:

The Respite Program Care Provider performs a variety of in-home services for home health patients to provide respite for their caregivers.

Responsibilities:

* Assists with implementation of the domestic plan of care including preparing meals, running errands, cleaning and doing laundry.

* Maintains accurate documentation in accordance with hospital policies and procedures.

* Communicates patient concerns or changes in patient status to supervisor.

* Provides assistance to patients with activities of daily living.

* All other duties as assigned.

Qualifications

Licenses/Certifications/Registration:

* Required Credential(s):

* Certified BLS Provider obtained prior to hire date or job transfer date.

* Chauffeur credentialed from the Michigan Bureau of Motor Vehicles obtained within 2 Months (60 days) of hire date or job transfer date.

Education:

* HS or Equivalent

* High School diploma or equivalent required and Work/personal experience in dealing with individuals with disabilities (physical and/or mental), chronic illness, and/or other special needs; in human growth and development with focus on adulthood to elderly stages; and in the effects of abuse and neglec
