Residential Service Coordinator

The Residential Service Coordinator will provide general case management and referral services to all HUD residents needing assistance. They will also coordinate a variety of services and activities for residents.

ESSENTIAL FUNCTIONS/TASKS:

* Provide general information and referral services to residents of HUD subsidized housing

* Maintain relationships with community partners and connect residents with partners

* Monitor the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.

* Educate and assist residents on availability of services, application procedures, lease, and House Rules

* Help the residents build informal support networks with other residents, family and friends.

* Work collaboratively with management and Supervisor of Service Coordinators to solve tenant issues and maintain housing stability of residents

* Coordinate recreational and educational activities for residents including but not limited to planning group outings, wellness and exercise groups, English language classes, holiday events, and monthly resident meetings

* Contribute to the writing and publishing of the building's monthly newsletter

* Maintain current documentation of resident services, progress notes, and program attendance

* Assist with the organization of demographic research including but not limited to semi-annual service survey, resident profiles, and relationship based demographic reports with assistance from all other staff

* Collaborate with all staff members to assist in the creation of HUD Properties policies

* Support HUD Properties Manager and others with projects and requests as assigned

* Keep HUD Properties Manager and others updated on vacancy status, tenant problems, building condition/concerns, budget, and compliance issues

* All other duties as assigned

EXPERIENCE, TRAINING, SKILLS REQUIRED:

* A minimum of 2-3 years' experience in social service delivery with senior citizens and people with disabilities

* Experience in coordination of social services

* Training in aging processes, mental health, and disabilities

* Training in social services, entitlement programs, aging and disability services.

* Ability to effectively communicate issues with residents, prospects, vendors, and co-workers.

* Proficient computer skills in MS Word, Publisher, Outlook, and Excel.

* Ability to take initiative in problem solving.

* Ability to efficiently manage time, set and maintain boundaries, and hold to Service Coordination ethical standards.

* Basic knowledge of Landlord Resident Laws, ADA requirements, Fair Housing laws, and OSHA regulations.

* Strong organizational skills and attention to detail.

* Dependable, reliable, cooperative.

* Good interpersonal skills and ability to build healthy working relationships with staff and residents.

* Strong work ethic and pride in work.

PREFERRED SKILLS AND EXPERIENCE:

* Experience in documentation of services and progress notes using Family Metrics or AASC Online

* Bilingual in English/Chinese, Farsi, Russian or Korean is preferred but not required

EDUCATION:

* A Bachelor's of Social Work or degree in Gerontology, Psychology or Counseling preferred. Some formal training and experience in lieu of degree.

LICENSE OR CERTIFICATE REQUIRED:

* Valid Driver's License.
