Medication Reconciliation LVN -ER

The Medication Reconciliation LVN works under the supervision of the Emergency Charge Nurse to assist in the process of obtaining accurate patient medication lists for each patient during admission, transfer, or discharge. Collaborates with physicians and staff to reconcile any discrepancies identified or seek consultation on medications, as needed. Maintains positive public relations by interacting with patients, visitors, physicians and other staff members in a friendly and professional manner and ensures the safe handling of infectious material(s) to protect all patients and employees.


* High School graduation or equivalent required.

* Completion of a Licensed Vocational Nursing Program with a current California LVN License required.

* Current Basic Life Support (BLS) Certification required.

* 1 year of recent experience on a Skilled Nursing Unit, Physician Office, acute care setting or pre-hospital acute care setting is required and successful completion of a pharmacology test.

* Recent experience in medication administration required.

* Comprehensive knowledge of current medications including brand/generic names and strengths strongly recommended.

* Must have good communication skills and be able to organize and prioritize patient care tasks effectively.

* Must have the physical and mental health sufficient to accomplish duties of a LVN.

* Must meet Marshall Medical Center's pharmacology requirements.
