Case Manager: Keeping Kids in School

Overview

The Keeping Kids in School Case Manager provides direct support to families referred by identified schools within Sonoma County for the Keeping Kids in School Project. They provide flexible case management services to children and families within continuity of care contexts, including advocacy for services within the community, social services system, educational and health system of services with emphasis in reducing truancy from school. Responsibilities

* Provide assessments and formulate case plans for caregivers, families, and school teams.

* Provide consultation and care plan coordination to families.

* Work effectively and collaboratively with a variety of public/private providers.

* Maintain appropriate record keeping and reporting requirements.

* Assist in data collection for further program development and research.

* Identify areas of need in the school and home that are contributing to truancy for the student and work collaboratively within the team and community to meet these needs.

* Offer direct client and family services (such as, procurement of legal services, emergency assistance, making appointments, court advocacy, respite assistance, etc.).

* Conduct Support Group meetings and other related support group activities to increase knowledge and self-help in the client group.

* Liaison between the family, community partners, and school team.

* Attend designated School meetings and Sonoma County meetings

* Conduct self in a manner reflecting credit on Seneca Center's Keeping Kids in School Program and encourage others to do the same

Qualifications

* Bachelor's Degree in social work or related field.

* Two 2 years minimum direct work experience providing support, community, or school based services to children, adolescents and families required.

* Strong computer skills

* Valid California driver's license and insurability by Seneca's insurance company

* Clearance of TB test, physical exam, fingerprints, and any other State/Federal requirements

Schedule

* Flexible schedule required with ability to work evenings and weekends as needed
