Hours: 12 hour shifts, every other week end and rotating holidays 7:00am- 7:30PM

Communicate changes in patient situations/conditions that would require notification to the RN. Assists with patient activities as delegated by the RN including activities of daily living (i.e. dressing, ambulating, toileting, eating, grooming, oral care and bathing.)Follows wound and skin care protocols to prevent skin breakdown and development of pressure ulcers. Observes and reports the presence of pressure areas and skin breakdowns to the RN and assists in carrying out plans to decrease pressure and skin breakdowns such as frequent repositioning. Observes for patient fall risk factors and communicates with RN for application of fall prevention strategies. Oversees fall prevention strategies for patients who have been identified as at risk. Performs safety rounding on each patient using department standards. Minimum interaction with the patient must include conversation about their need for toileting, positioning, pain control, and possessions. Utilizes the pain scale when performing rounding and as needed. Reports all verbal or non-verbal pain behaviors to the RN. Under the direction of the RN offers non-pharmacological pain intervention to assist with patient comfort. Obtains pertinent data pertaining to the patient as delegated (i.e. blood pressure, pulse, pulse ox, temperature, input and output), accurately documents care provided and notifies RN of outcomes in a timely manner.


* Certified nursing assistant on the Wisconsin Nurse Aide Registry (current or expired)
