POSITION DESCRIPTION

The incumbent is responsible for providing technical and professional information and advice: counseling and treatment for patients and their families. The incumbent will provide epidemiologic investigation and intervention to control the spread of disease. The incumbent is under the direct supervision of Community Health Nursing Consultant. The incumbent is required to have a reliable source of personal transportation to fulfill the responsibilities of this position. An RN license in good standing is required for this position. This is a Grant-funded position.

Manages a reportable disease patient caseload, including interviewing patients, conducting field investigations, identifying other persons at risk; performing case analysis, and preparing documents relative to the epidemiological or disease control process.

Provides disease intervention services. Makes recommendations and implements control measures per FDOH- Marion County standard operating procedures, state guidelines and statutes, and CDC guidelines. Confers with immediate supervisor, director of nursing, and/or FDOH- Marion County administrator on recommendations as needed.

Conducts follow-up interviewing, investigation and other intervention procedures. Acts as client advocate to assist coordination of visitation to medical provider and laboratory offices, hospitals, schools, or other sites of illnesses and/or outbreaks, monitors infectious disease reports, and provides follow-up on positive lab results with private medical providers in the community. Conducts case investigations which may include emerging and highly infectious diseases.

Collaborates with other health department staff, infection control practitioners in hospitals, nursing homes, and other facilities for the identification, investigation, and control of infectious diseases. Interacts with the public and private sector and other community partners as well as other health departments of needs/concerns being expressed by the public/private sector. Performs data entry in Merlin, HMS, FL Shots and other appropriate databases.

Provides nursing services to include, but not limited to, patient assessment, injections of vaccinations and immune globulin, venipuncture, medication administration, collection and processing of laboratory specimens, and patient education. Records all nursing services provided in electronic health records as required by meaningful use requirements. Maintains appropriate epidemiology case investigation records and documentation.

Provides after hours and weekend vaccinations for rabies patients when needed per rabies vaccine schedule.

Perform other related duties, such as, but not limited to, working where assigned at the time of a disaster.
