NYS ACHIEVE (AdvancingCare through Health Integration and EVidence-based Effort) is a collaborativeapproach, which will integrate the High Fidelity Wraparound (HFW) model withNYSs Medicaid Health Homes Serving Children program for youth and young adultswith Serious Emotional Disturbance (SED), ages 12 to 21 years, with high andcomplex needs including out-of-home placement or risk of placement, and theirfamilies. At the local level, a triadteam will serve each child/family and will be made up of a Family PeerAdvocate, a Youth Peer Advocate and a HFW Care Manager.
The ACHIEVE Family Peer Advocate (FPA) will function as a member of theteam and deliver Family Peer Support Services, which are an array of formal andinformal services and supports provided to families caring for/raising a childwho is experiencing social, emotional, developmental, medical, substance useand/or behavioral challenges in their home, school, placement and/or community.Family Peer Advocates provide a structured, strength based relationship withthe parent/caregiver for the benefit of the child/youth.
A Family Peer Advocate will:
engage the child/family, with other triadmembers, to begin the partnership and connection to delivery of strengths-basedsupports and services using the HFW model;
conduct a family/caregiver assessment todetermine family strengths and needs, to inform the child/youths CANS-NY andcomprehensive assessment;
participate in, and facilitate when appropriate,child and family team meetings;
carry out specific tasks with the family/caregiverto assist the family/caregiver in meeting the child/youths plan of care goals;
provide the following service components
o Outreach and Information:
Empower families to make informed decisionsregarding the nature of supports for themselves and their child.
o Engagement, Bridging, and Transition Support
Provide a bridge between families and serviceproviders, support a productive and respectful partnership by assisting thefamilies to express their strengths, needs and goals.
o Self-Advocacy, Self-Efficacy, and Empowerment:
Coach and model shared decision-making andskills that support collaboration, in addition to providing opportunities forfamilies to self-advocate.
o Parent Skill Development:
Support the efforts of families in caring forand strengthening their childrens mental, and physical health, development andwell-being.
o Community Connections and Natural Supports:
Enhance the quality of life by supporting theintegration of families into their own communities.
Complete NYS ACHIEVE required training on HighFidelity Wraparound and the project guidance;
Complete all required documentation of theproject;
Participate in regularly scheduled supervision;and
Meet requirements for continued education tomaintain FPA credential.
