The Skilled Nursing Facility Practice Clinical Supervisor (SNF CS) is responsible for the supervision of patient-centric clinical care that results in clinical excellence. The CS is responsible for holding clinical staff accountable for achieving job expectations:

* Productivity standards

* Scheduling timelines and responsibilities

* Case management functions

* Communication and coordination of services

* Documentation standards

* Continuity standards

The CS is responsible for effective and efficient patient-centered care through weekly huddles with clinical mission partners and through the oversight of scheduling communication exchanges between the practice and partner long term care facilities. The CS also assists in the daily communication and operations of the practice, serving as back up to clinical staff as needed.

Additionally, the CS is responsible for variance reporting of key results and initiating action plans to mitigate variances.

The CS acts as a liaison to staff development, compliance/risk, facility partners surrounding standardized department/division processes and staff competency. The CS is expected to participate in monitoring activities to support improvement in process and clinical practice.

The CS reports to the Director, SNF Practice and supervises clinical staff within the practice, including clinical liaisons and clinical coordinator. The CS will assume responsibility for the practice in the physical absence of the Director.

REQUIRED QUALIFICATIONS:

Currently licensed as a registered nurse in the state(s) in which they are practicing

BSN required by 12/31/2020 if does not currently have.

Previous supervisory experience or minimum of 2 years of progressively more responsible related work experience.

2-years recent nursing experience required; long term care environment preferred.

Demonstrates excellent communication, human relations, and analytical and problem solving skills.

Demonstrates ability to work independently and execute own workload.

Demonstrates critical thinking skills.

Excellent communication skills.

Strong interpersonal skills necessary to facilitate learning of others and provide effective leadership for departmental personnel.

Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.

Maintains a valid driver's license.

Must have certification in CPR prior to patient contact.

Must be able to use and operate medical and/or office equipment/computers, including Point of Care devices.

Knowledge of reimbursement and regulatory requirements for Skilled Nursing Facilities.

Participation in On-Call duty rotation.

Occasional weekend duty as needed to fulfill role with weekend staff members.
