ESPRI Care Coordinator

JOB SUMMARY: The person in this position will be the lead Care Coordinator for the Niagara Falls Empire State Poverty Reduction Initiative and will be responsible for working with participants and partner organizations to ensure success of each individual in the program. They will be responsible for exercising considerable initiative and independent judgement in the performance of duties.

ESSENTIAL FUNCTIONS:

* Act as primary coordinator of social and educational services for participants of the Empire State Poverty Reduction Initiative in Niagara Falls

* Initiates contact with the client, accurately assesses service needs and follows up

* Obtains information to evaluate and to accurately develop plan of action

* Possesses well developed interviewing skills

* Defines problem areas and demonstrates incumbent's role in plan of action

* Makes appropriate referrals and follows up

* Takes the initiative where appropriate, such as reach-out advocacy

* Maintains a working relationship with community resources for accurate service delivery

* Develops other resources as needed

* Disseminates information regarding agency purpose

* Participates in community affairs which relates to agency programs and goals

* Implements agency policy and procedure

* Takes leadership role on special agency committees

* Meets quantitative expectations established by the agency

* Supervises volunteers, undergraduate/graduate students

* Performs other duties as assigned

* Comply with the Agency's Compliance Program, Code of Ethics, laws, regulations, and ethical standards applicable to your job duties.

* Knowledge of programs and services that exist in community

* Excellent time management skills and able to organize and manage large amounts of flies, schedules, and information

* Comfortable in a fast-paced environment with multiple cases

* Self-directed and able to work without supervision

* Demonstrates compassion, patience, empathy, and open-mindedness

MINIMUM REQUIREMENTS:

EDUCATION: Successful completion of the requirements for a Master's Degree of

Social Work, Counseling, or related field from an accredited college or university, or completion of 60 hours towards graduate degree.

EXPERIENCE: One or more years of experience in the fields of Social Work or Counseling, or related field.
