The Universal Care Partner CNA works under the general supervision of the ADHC Manager and/or Home Care Manager. The position performs tasks both in the PACE Day Center and in the PACE Participant's home which support the social, physical and cognitive functioning of seniors with impairments. The Universal Care Partner demonstrates the knowledge and skills necessary to provide an/or assist with appropriate care to include: personal care (feeding, grooming, bathing, toileting), home services to include laundry, memory stimulation activities (social, recreational), motor activities, escorting to medical appointments, shopping, assisting with transportation of participants to and from the facility utilizing the facility transportation vans and other activities as directed by the Participant's plan of care (POC). Assists with any identified needs of the Participant during their onsite stay at the PACE facility and in their home that will enable the Participant to perform at the greatest level of independence possible.

Greets and assists participants and their family / caregivers upon arrival to and departure from the facility in a welcoming and caring manner.
Performs nursing care duties as directed by participant's plan of care and/or as directed by the Home Care, Clinic Nurse Managers or by the assigned RN and/or PT to include taking vital signs (blood pressure, weights, temperature and blood sugar at scheduled times) and accurately completing all documentation of results. Also provides assistance with the following: application of lotion, measures intake and output as ordered, obtains stool specimen, empties/changes Foley catheter drainage bag, and applies TED stockings. Compassionately engages participants in activities, conversations, recreational activities, and motor activities in both the facility and home environments to include restorative therapies and activities. Accepts guidance and instruction from members of the PACE Clinical teams and/or management involving Participant care.
Ensures availability of resources (equipment, instruments, and supplies) necessary to ensure positive patient outcomes. Assists with cleaning and maintaining order of assigned patient care areas to include showers, bathrooms, kitchen and activities area so that the facility and / or home environment is clean, safe and supportive for the Participants. Responsible for linen changes in the facility as needed as well as in the homes.
Completes assigned personal care tasks according to Participant's plan of care and/or schedule to include shower/bathing; grooming, toileting and feeding in a thorough manner with the utmost attention to quality and safety of care provided and that maintains the confidentiality of patient/customer information as required by InnovAge PACE policy and HIPAA regulations.

Must be able to exhibit competency before performing personal care / aide services; before rendering services to Participants at home, the Universal Care Partner (UCP) must be checked off on a skills check list by a Registered Nurse. UCP will not be able to render any service which he/she has not successfully demonstrated competence and will not be able to render any care if more than one of the core skills are not demonstrated at the appropriate skill level. Must meet the standards set at 42 CFR 484.36

High School Diploma or GED
Licensure, Certification, Registration or Designation
Current and valid state issued Certified Nursing Assistant (CNA) license is required
Current CPR certification or the ability to obtain CPR certification.
Require personal transportation, current state issued driver's license, good driving record and auto insurance as required by law
Work Experience and Qualifications
A minimum of one year experience working the frail or elderly is required.
