Job Title: Social Worker – Medicine Surgery

Roles & Responsibilities:

The social worker in this position is one of two social workers providing psychosocial services with an emphasis on discharge planning for patients on this medical teaching unit. The emphasis is on helping patients and families dealing with the impact of acute illness. Assignment may include some critically ill stepdown patients. There is a strong emphasis on complex discharge planning. The social worker will have the opportunity to collaborate as a member of the team, which requires strong interdisciplinary skills, as well as knowledge of crisis, and family systems. Coverage will include other medicine/surgery units.

Requirements:

New York LMSW.

Excellent clinical, collaborative, and organizational skills.

Knowledge of crisis intervention and ability to work with multiple hospital and community systems.

Inpatient medicine/surgery hospital experience preferred.
