Occupational Therapist - PRN

OT shall screen, evaluate, re-evaluate, discharge plan and make revisions in any established plan of care as supervised by the physician.


* Must have a degree in Occupational Therapy from an accredited program.

* Must possess a current certification from NBCOT

* Active state license required.

* Excellent organizational and time management skills.

* Excellent written and verbal communication skills.

* Possess current working knowledge of company policies and procedures, clinical programs with application skills to therapy disciplines.

* Active CPR certification.

* Annual TB skin test required

* Ability to supervise COTAs and OT Techs.


* Screen all residents in facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Occupational Therapy intervention is indicated.

* Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.

* Re-assess patients and caseload at least every 30 days in order to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.

* Supervise treatment implementation carried out by Certified Occupational Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated.

* Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.

* Educate staff, family and care givers as indicated.

* Identify needs for program development and implement programs according to company policies and procedures.

* Identify equipment needs of Occupational Therapy Dept. and order equipment according to facility policy.

* Complete all required documentation according to guidelines specified in Documentation Manual.

* Coordinate in-service for facility staff as need is determined, to ensure safe and effective carryover of Maintenance/Restorative Programs and overall call of the residents.

* Attend all facility required meetings.

* Promote profession within facility, including assisting with coordinating special events i.e. National Occupational Therapy Month.

* Attend Continuing Education courses specific to current position

* Maintain professionalism and follow all national/state associations regulations at all times.

* Supervise student affiliates as directed
