POSITION SUMMARY:

Assists with coordination of home care at The Residence at North Ridge. Assesses and supervises home care for residents. Consults with home health staff regarding any change of condition concerning residents. Performs all nursing duties as requested. Follows established state regulations regarding home care. Must be pleasant, respectful and courteous to residents, families, visitors and fellow employees at all times.

MAJOR JOB FUNCTIONS:

Provides nursing care to residents.

* Conducts initial interview of resident(s) and assesses for home care, or refers to alternative setting if needs cannot be met.

* Assess resident(s) change of condition, including emergencies, acute and chronic medical conditions and updates service agreements and care plans accordingly.

* Initiates service agreements and care plans for residents requiring home care services.

* Ensures care plans are being followed in an appropriate and timely manner.

* Coordinates move in proves and initiation of services prior to move in and ongoing.

* Communicates residents condition as appropriate, to resident, family, staff, physicians and involved care team.

* Refers skilled nursing care to Medicare certified agencies.

* Delegates nursing tasks to staff who are competent to perform the procedures that have been delegated.

Supervision of staff.

* Responsible for direction and supervision of home care staff.

* Conducts orientation of all new employees to department policies and procedures.

* Completes and conducts LPN and HHA job performance evaluations.

* Instructs the Orientation to Home Care/Home Health Aide Competency Evaluation Program.

* Creates and reviews home care policy and procedures and communicates changes to home care staff.

Reports and records resident information.

* Ensures that all services are properly documented.

* Receives and processes physician orders.

* Maintains health record and miscellaneous duties.

* Reports progress and information to Director of Wellness.

Conducts miscellaneous duties.

* Responsible for upholding regulations of Home Care Licensure.

* Conducts ongoing education for residents, families, and staff as required/requested.

* Attends appropriate meetings.

* Participates in family conferences as requested.

* Participates in interviews of potential staff.

* Assists Director of Wellness on projects as assigned.

* Performs all other duties as assigned.

* Assists with staffing/scheduling

* On call coverage rotation

* Other duties as assigned

JOB SKILLS, KNOWLEDGE AND ABILITIES:

* Current Minnesota RN license (prefer bachelor's degree).

* Current CPR certification.

* Strong knowledge of state and federal regulations concerning home care.

* Good technical knowledge of needs and illnesses common in the elderly.

* Knowledge of home care-assisting options within the community.

* Two years experience in geriatric nursing.

* Prefer one year supervisory experience.

* Must be able to read, write, and speak English.

* Must be able to perform Physical Job Demands of position.

* Exercises confidentiality to ensure compliance with all HIPAA regulations.
