The Physical Therapists (PT) uses his or her clinical expertise to plan the delivery of safe therapeutic, age appropriate patient care through assessment, planning, implementation and evaluation over a defined scope of practice. Physical Therapy, as provided by the Home Care Physical Therapists, is evaluation of the degree of disability and capabilities of all patients of all ages and administration of treatment on an intermittent basis at their home to correct and/or minimize disabilities.

Minimum Requirements:

Education: Bachelor's Degree of Certificate of Proficiency in Physical Therapy.

License/Certification: Satisfactory proof of New York State Licensure as a Registered Physical Therapist

Experience: Minimum one (1) year experience as a Physical Therapist. Home Health experience preferred.

Skills: Knowledge of the range of treatment needed by patients as appropriate to their age. Communicate effectively. Good interpersonal skills necessary to interact with patients, families and other employees and members of the health team.

Other: Transport self to patient's homes under all weather conditions. Negotiate architectural barriers in patient's home, and to work within the environment of care.
