Responds to requests of former patients, physicians, hospital staff and guest for health information while preserving the confidentiality of patient's protected health information for BHSF facilities. Performs quality assessments reviews on release of health information completeness and accuracy by meticulously checking for all required components of requests received for protected health information. Answers phone calls and provides information adhering to hospital components of requests received for protected health information. Answers phone calls and provide information adhering to hospital disclosures of Medical Record Information policy and State and Federal law and HIPPA regulations. Calculates prepayment notices for billable requests exceeding $25.00. Works as a team to meet Release of Information and department goals.

Answers phone inquiries about release of information and provides information adhering to hospital Disclosure policy, State law and HIPAA regulations while maintain BHSF telephone etiquette standards.

Verifies that proper authorization has been obtained for release of information following State and Federal Law, and HIPAA regulations with a 98% accuracy rate.

Assures all requests for internal and external customers are timely and properly released. Stat continued care internal and external customers will be process within 1 hour of receipt. Non stat continued care will be process within 24 hours of receipt with a 98% accuracy rate.

Performs quality assessment reviews ensuring completeness and accuracy of release of information requests accounting for super sensitive information (i.e. HIV, mental health and substance abuse) with a 98% accuracy rate.

Prints, highlights, and matches master patient index to request with a 98% accuracy rate.

Returns timely any request that cannot be processed due to improper authorization.

Properly prints from the online record system and or orders from off site storage or other departments that maintain health records that are needed. Reviews records requested to determine that the component parts of the medical record are complete and accurate; following up with corresponding auxiliary department for needed documentation with a 98% accuracy rate.

Properly and timely faxes, emails or produce CD/thumb-drive from the online record system(s) that maintain health records that are needed. Accurately calculates if needed and issues prepayment notices for billable requests exceeding $25.00 with a 98% accuracy rate.

Works as part of the ROI team to achieve productivity standards by supporting Department goal of not having pending request for release of information over 225 requests for all facilities with turn around time of 3 business days from receipt of request.

High School, Certification, GED, Training, or Experience

Knowledge of medical terminology, clinical chart format, and computer skills. Ability to work in a highly focused customer service oriented setting with high volume telephone experience.

Excellent communication skills both written and verbal

Must be attentive to fine details and be a high volume performer with exceptional organizational skills

Typing (25 wpm), Word, and Excel testing required
