Definition:

The Social Work Assistant provides direct intervention for the social and emotional needs of the patient, under the supervision of the MSW.

Line of Authority:

Masters Social Worker/LCSW; Administrator/Director of Services

Qualifications, ALL:

* Graduate of an accredited college or university with degree as described below.

* At least two (2) years experience as a social work assistant required; one (1) year of in a health care setting preferred

* Has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service, ONLY with respect to persons initially licensed by a State or seeking initial qualification as a social work assistant BEFORE after December 31, 1977.

TENNESSEE Specific

* Baccalaureate degree in social work, psychology, sociology or other field related to social work (bachelor/masters degrees in psychology, sociology, human services (behavioral sciences, not human resources) OR

* Master's degree in counseling fields (psychological guidance and guidance counseling) and degrees in gerontology.

SOUTH CAROLINA Specific

* Baccalaureate degree in social work, psychology, sociology, or other field related to social work,

* Current licensure as Medical Social Worker Assistant by the South Carolina Board of Social Work Examiners.

FLORIDA Specific

* Graduate of an accredited school of social work with one year of experience in social services

* To provide clinical counseling to patients or caregivers, the social worker must have a current state license as a Licensed Clinical Social Worker (LCSW), pursuant to Chapter 491, F.S.

MISSOURI Specific

* Baccalaureate Level Social Workers with current licensure as Licensed Baccalaureate Social Worker (LBSW) or Licensed Baccalaureate Social Worker, Independent Practice (LBSW-IP).

Performance Requirements:

* Able to speak, read, write and follow written orders.

* Able to see and hear adequately in order to respond to auditory and visual requests.

* Able to carry out fine motor skills with manual dexterity.

* Able to speak in a clear, concise voice in order to communicate adequately.

* Able to handle emotionally stressful situations on a regular basis, including but not limited to death and dying.

* Able to organize work procedures

* Able to learn, absorb and apply professional training.

* Must have reliable personal transportation.

Specific Responsibilities:

* Provides social services under the supervision of the MSW/LCSW and according to physician order and homecare policies and procedures. Services are focused on minimizing the negative impact of identified social and emotional issues on the patient's ability to recover

* Documents services as provided.

* Participates in inservice training for homecare staff related to social services.

* Contributes to applicable QI activities as requested.

* Serves as Utilization Review and/or Advisory Board member as requested by Administrator/Director of Services.

* Completes other duties as assigned by the MSW/LCSW/LISW/the Administrator/ Director of Services

* Maintains knowledge base of home care social services procedures and regulations as well as professional standards through attendance at workshops and inservices, and individual research and reading.
