NATURE AND SCOPE OF POSITION

The RN I is a graduate nurse with an Illinois State License. The RN I, novice to advanced beginner, provides nursing care to acute care patients while collaborating with the nursing care team and multidisciplinary team to advance the patient plan of care. Nursing practices is guided by the American Nurses Association (ANA) Scope and Standards of Professional Nursing Practice and Code of Ethics. The RN I uses guidelines, policy, and procedures to guide interventions for patient care while gaining the experience in understanding context and analytical reasoning in the clinical setting. The RN I willingly collaborates with more experienced members of the multidisciplinary team to plan patient care.

This position adheres to and is responsible for the safety standards of Centegra Health System including the completion of the mandatory organizational safety standards on an annual basis

Identification of population served by this position: Neonate through Geriatric as determined by department scope of service.

PRINCIPAL RESPONSIBILITIES

Application of the nursing process

* Assessment: The RN I assesses the healthcare needs of patients/families by completing a physical and psychosocial assessment of patient/family condition. Includes pertinent data in assessment.

* Demonstrates knowledge regarding the biological, psychosocial, ethnic and religious needs of the patient population, in addition to the age-specific learning needs, and incorporates those needs into the individualized plan of care.

* Diagnosis: Analyzes assessment data to determine potential nursing diagnoses/patient problems.

* Outcomes Identification and Planning: Collaborates with patients/families, nursing care team, and multidisciplinary team to identify expected outcomes/goals of care for each patient and contributes to care planning to achieve individualized patient goals. Recognizes patient/family as an integral member of the healthcare team.

* Implementation: RN I implements the identified plan of care by providing evidence-based interventions. Coordinates patient care for assigned patient load seeking assistance in the planning and implementation phase as needed resulting in satisfactory outcomes. The RN I identifies elements of care that are appropriate to delegate to unlicensed assistive personnel. Coordinates patient care needs, including discharge planning, with appropriate departments.

* Evaluation: Evaluates effectiveness of plan of care by assessing patient/family response to interventions. Collaborates with nursing care team and multidisciplinary team to alter plan of care as needed based on patient response.

Professional Performance

* Practices ethically and maintains confidentiality of patient information. Identifies situations that require ethics consultation/review. Reports safety and ethical concerns using the organizational processes for reporting.

* Maintains knowledge and competency to reflect current nursing practice including basic knowledge of the nursing process. Completes mandatory education as identified by Centegra Health System. Evaluates own practice and establishes professional development goal(s) every year and develops a professional development plan to achieve goal(s). Willingly gives and receives feedback related to nursing practice in the practice environment.

* Communicates clearly to members of the nursing care team and multidisciplinary team using established communication tools to support safe and effective handoffs, transfer of information, and delegation to other members of the care team. Effectively documents care plan and patient care.

* Demonstrates leadership in the care of assigned patients. Maintains responsibility for delegated tasks. Collaborates with patient/family and healthcare team.

* Demonstrates financial stewardship by using resources wisely and in an environmentally safe and healthy manner. Identifies opportunities to improve resources or resource utilization.

* Demonstrates a gradual increase in independence in clinical practice.

* Demonstrates compliance with institutional and department policies and procedures using available resources.

This position performs other duties as assigned.

MINIMUM KNOWLEDGE, SKILL AND PHYSICAL ABILITIES REQUIRED

Experience

Successful completion of required clinical rotations at an accredited college of nursing

Some patient care experience as a nursing assistant or similar role

Education

BSN preferred, ADN minimal requirement

BSN, ADN candidates must agree to complete BSN within 5 years of hire date

Certification

BLS (specialty areas may define additional certification requirements)

BLS and ACLS (specialty areas may define additional certification requirements)

National certification in area of specialty (ANCC recognized certification)

Licensure

Illinois Registered Professional Nursing License

Physical Ability to perform functions of the job
