Position Overview: The Case Manager assists elders in obtaining services that will secure an independent lifestyle by identifying needs and authorizing services that will maximize independent functioning.

Responsibilities include: Conduct an initial assessment of physical, social, environmental and emotional status to determine needs and eligibility requirements within the established time frame guidelines. Develop service plan and supportive networks through contact with client, formal and informal supports, and consultation with supervisor. Assist client to obtain and utilize other community services such as: VNA, housing, fuel assistance, council on aging, Medicaid. Monitor client needs and service plan on an ongoing basis. Provide support and information to client and those directly involved in clients care. Complete authorizations, notices of suspension, and other related forms in a timely manner. Record statistical data. Accurately complete monthly reports.

Bachelors degree or equivalent experience. Knowledge or previous experience with State Home Care or Senior Care Options program a plus. Successful candidate will have reliable transportation with a willingness to frequently travel throughout assigned areas.

Compensation and Benefits:

This is Monday-Friday, 35 hour-per-week position with competitive pay, mileage reimbursement, excellent work environment and an outstanding benefits package, including health, dental, vision, company paid life insurance, and health reimbursement account. Additionally, this position offers a retirement plan, generous paid time off, and employee referral bonuses. Upon Hire you will receive 6 days of personal time and accrue vacation immediately. $1,000 hiring bonus is payable in two payments of $500 upon successful completion of 3 and 6 month orientation periods.
