Summary

HOURS: Full-Time; Monday - Friday 8:00AM - 4:30PM.

POSITION SUMMARY: The Care Connections Program Coordinator works closely with the administrative and medical leads of the Care Connections program. Responsible for the coordination of administrative duties related to program development, implementation, and outcome measurements. Trains and supervises care management personnel within the program.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

* Collaborates with management regarding defining project scope, goals, and deliverables that support project goals.

* Uses stratification methodologies to identify and target appropriate populations for Care Connections. Includes monitoring and facilitation process for enrollment.

* Facilitates Care Connections team meetings (including North/South team meetings, work groups, etc.) to support Care Connections activities.

* Collaborates with practice supervisor to ensure appropriate billing and office procedures per regulatory bodies.

* Maintains Care Connections recommendation system.

* Trains and provides day-to-day informal supervision of program care management personnel.

* Represents Care Connections in regional collaborative meetings.

* Conducts and oversees quality improvement activities as determined by annual and long-term goals, including but not limited to program, process, and outcome deliverables. Will provide monthly review of KPIs.

* Collaborates with system-wide leadership to share learnings across the health system.

* Participates in LG budget process including monthly revenue reports and attendance at various budget meetings.

* Maintains Advance Care Planning instructor certification for first and last steps conversations.

* Leads team-care events such as clinical supervision and Balint groups to facilitate team self-care.

MINIMUM REQUIRED QUALIFICATIONS:

* Bachelor's Degree in a related field is required.

* Two (2) years previous supervisory experience is required.

* Prior program development experience is preferred.

This position requires PA Child Abuse and FBI Fingerprinting Clearances.

This position is located in Lancaster, PA.
