The Patient Care Navigator works under the direction of the Director Breast Svc & CRPP and supports Breast Care Supervisor in implementation of service for breast program for clinical navigation, MRI, Cancer Risk and Prevention Program and associated activities for diagnostic and screening care. The position has direct accountability for staffing scheduling support to meet patientcare and customer service satisfaction metrics, standardize workflows with accountability to train and coach as needed, responsible for navigating the daily patient flow, including insurance authorization aspects of patient visits, review of medical records related to the clinical recommendations and/or referrals to the MRI breast service program, including support of risk analysis documentation through the Cancer Risk and Prevention Program (CRPP), related referrals, and patient care navigation and facilitation with oversight directly to outcomes of Patient care scheduling metrics and staffing of support scheduling team. Other responsibilities include provision of organization, communication and assertive skills necessary to facilitate patient activities in coordination with ancillary departments. Identifies and resolves discrepancies related flow process of patient care. Also, ensures adequate inventory of medical and office supplies. Accountability include collaboration with the healthcare team towards achieving fiscal, quality care goals, and data outcomes for breast care program services and Cancer Risk and Prevention Program.
- High School graduate or equivalent required
- Medical Assistant Certification preferred
- American Heart Associate BCLS preferred
- Breast Navigator Certification preferred
- A.A or A.S Degree preferred
