ACT Team Lead Mental Health Therapist

Seeking a mental health therapist to lead the Acceptance and Commitment Therapy (ACT) team. This position will monitor and track the implementation of ACT, analyze services both quantitatively and qualitatively (especially related to maintaining ACT fidelity), and lead team that provides therapy and case management services, including but not limited to: providing support to maintain the consumer's preferred level of independence and community integration, psychosocial skills training, and supportive counseling to individuals with mental illness and their families.

Essential Duties:

* Support ACT team members through training, coordination of program, advising, and recruiting and retaining members.

* Track and report on data collected including supported employment, weekly contacts, level of care, etc., and submit required GOHBI and State of Oregon reports.

* Coordinate consumer access to ACT team services.

* Provide direct services to individuals enrolled in the ACT program, including individual and group therapy, case management, mediation deliveries, care coordination and skill building.

* Assist consumers with medication management, monitor medical issues, and document services provided.

* Work closely with consumers and their families, and community partners to obtain services for consumer including financial and housing assistance, transportation, mental health services, medical services, social services, personal care assistance and educational services.

* Facilitate and conduct training sessions to assist consumers in maintaining or achieving desired degree of independence.

* Develop, implement, and maintain behavior plans and interventions based on the principles of Positive Behavior Support.

* Coordinate and supervise or facilitate peer support, IMR/EIMR, and A&D groups and ensure ACT fidelity requirements are met.

* Run or delegate one A&D group

* Oversee ACT admission and graduation process and determine level of intervention necessary.

* Set annual ACT program goals.

* Comply with HIPAA, Code of Federal Regulations and all other applicable laws.

* Facilitate daily team meetings, and monitor and update meeting notes to ensure consumer receives medically necessary services.

* Be accessible as the 24/7 crisis worker for ACT, or delegate.

* Research and develop community collaboration opportunities to improve program and consumer's quality of life.

* Must maintain punctual and regular work attendance.

* Must have the ability to maintain positive and professional working relationships with co-workers, clients and other agencies.

The Requirements:

* Master's degree in a social sciences field.

* State licensure or be willing to pursue licensure preferred.

* Two years of relevant mental health experience.

* Must meet requirements of a Qualified Mental Health Professional.

* Certified as an Alcohol and Drug Counselor.

* Knowledge of practices, principles and philosophy of mental health treatment, including motivational interviewing, assessment, treatment plans and diagnoses, and crisis counseling.

* Knowledge of computer basics.

* Communicate effectively verbally and in writing.

* Proficient in coordinating services with care providers from other agencies.

* Ability to provide adequate written documentation of services rendered in order to satisfy federal, state and ACT requirements.

* Ability to develop documents, spreadsheets, and presentations that illustrate and track how ACT services are being implemented and improved.

* Valid driver's license, and must pass criminal history check and drug screening.
