Our Service Coordinator is responsible for ensuring all aspects of the individualized treatment plan is age-appropriate, need-related and person-centered utilizing the individuals strengths, choices and interests. The successful candidate will also act as a facilitator of the Interdisciplinary Team to integrate, coordinator, implement, monitor and adjust as needed, each individual's plan of active treatment. Requirements Bachelor's degree in a human services field (including, but not limited to: sociology, special education, rehabilitation counseling and psychology) and two (2) years of experience working directly with persons with mental retardation or other developmental disabilities.
