Case Manager - Idaho Falls

Seasonal full-time position (season usually runs May - October each year)

Essential Duties and Responsibilities (Other duties may be assigned)

* Evaluate, analyze, plan, implement, and coordinate extensive health, dental, and nutrition service for enrolled children

* Collaborate with community partners, parents and staff to ensure that all enrolled children are up-to-date with their age appropriate preventive screenings following the EPSDT schedule and by coordinating services for identified health-related treatments

* Promote, plan, coordinate, assist, and facilitate educative trainings and meetings following MSHS parent curriculum, Center Health Service Advisory Committee (HSAC), center parent meetings, and UCLA Health Literacy Program for parents

* Assist children and families in obtaining health, education, early intervention, advocacy, social services, and other services under state, federal, and other applicable regulations

* Collect and track confidential data to maintain accurate, detailed, and most current records for health, dental, and nutrition screenings and assessments

* Monitor individual case requirements, compile data, referrals, and follow-ups to regularly discuss the status with Center Management Team during Family Advocacy Meetings and as needed

* Establish community partnerships by maintaining regular contact to stay current on new services, to advocate, and refer enrolled children when a need or suspect of health, dental, or nutrition concern is identified

* Attend, participate, and facilitate at local network and group committees to serve as an advocate for MSHS program for health, dental, and nutrition needs of migrant farm workers

* Learn and follow MSHS policies and Head Start performance standards regulations to ensure its compliance

* Coach, mentor, and facilitate health-related service trainings to staff and volunteers at local trainings, orientations, and as needed

* Attend and participate in MSHS trainings and in other education, or professional development trainings, webinar or classes

* Ability to multi-task, manage time, meet deadlines and remain calm and professional under pressure

* Maintain standards of confidentiality of CC Idaho clients and records

* Perform all work duties and activities in accordance with CC Idaho policies, procedures, and safety practices



* High School Diploma or GED

* One or more years in community based services or related experience

* Current driver's license and proof of auto insurance

* Ability to read, speak, and write in both English/Spanish in a business setting

* Must pass physical examination and background checks, including state day care clearance

* Must obtain clearance of Tuberculosis

* Must obtain First Aid and CPR certification


* Associate degree with early childhood education emphasis, social services, or related health field
