Medical Assistant - Patient Enrollment Representative

The Medical Assistant - Patient Enrollment Representative will serve as the CareSync representative responsible for educating and enrolling eligible patients into CareSync's Care Coordination programs. The ideal candidate has experience working in a medical office environment, is passionate about providing excellent service and care, is organized, flexible, and has the ability to work well with a diverse team and population.

Duties and Responsibilities:

* Educate patients and caregivers on the value of CareSync's care coordination services

* Identify patients who are eligible for CareSync services

* Enroll eligible patients, referred by their healthcare provider, and assist them in setting up their CareSync account

* Gather important information to onboard the patients and properly set expectations

* Perform accurate and timely data entry

* Direct patient inquiries to appropriate clinical staff when appropriate

* Serve as an onsite liaison between CareSync and the medical facility to ensure client satisfaction and ongoing program success

* Act as a guardian of high-quality medical data and have concrete knowledge of state and federal guidelines for the Health Insurance Portability and Accountability Act (HIPAA) and other privacy standards

Knowledge, Skills and Abilities Required:

* Strong typing skills and computer proficiency

* Excellent organizational, communication and problem-solving skills

* Attention to detail

* Exceptional follow-up and interpersonal skills are essential

* Ability to be flexible with constant realignment of priorities, workflows and procedures to provide the most effective enrollment and patient engagement

* Ability to manage project tasks and timelines in a fast-paced environment

* Must be able to work independently in compliance with CareSync policies and procedures to meet team objectives in a remote setting

Education and Experience Requirements:

* High school diploma or equivalent

* Medical Assistant, preferred

* 2 years medical office experience
