Case Manager RN

Job Description

This exciting opportunity is based out of the Hartford, CT or Albany, NY office.

Telework available for Aetna employees who are currently WAH.


The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit design and/or acute and chronic health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN with unrestricted active license.

Fundamental Components:

* Understanding Clinical Impacts

* Providing Solutions to Constituent Needs

* Providing education to promote health engagement

* Working closely with members to implement an individual plan of care


* 3-5 years clinical practice experience required

* Case management experience preferred

* RN Licensure required

* Strong computer skills including toggling with Windows, Microsoft Office and Outlook and strong keyboarding skills

* Ability to managed multiple priorities, effective organizational and time management skills

* This position is an in office environment using a computer station with dual monitors and may require sitting for extended periods of time

* Excellent communication skills


The minimum level of education required for candidates in this position is an Associate's degree or equivalent experience.


RN Registered Nurse License is required

Nursing/Certified Case Manager (CCM) is preferred
