Job Requirements:

Physician - Family Practice Job Overview:

  • Perform a full range of physician services for all ages and genders
  • Direct, perform, or assist in the instruction of other health care professionals within the scope of the clinical privileges or responsibilities, to include providing oversight of mid-level providers.
  • Examine, diagnose, treat or prescribe courses of treatment within the scope of training, experience, and privileges.
  • Provide preventive and health maintenance care, including annual physicals, positive health behaviors, and self-care skills through education and counseling.
  • Order diagnostic tests as applicable as well as request consultation or referral with appropriate physicians, clinics, or other health resources as indicated.
  • Technically proficient in directing and teaching other medical staff, providing educational lectures and participating in the provision of in-service training to clinic staff members.
  • Deliver care within the patient-centered medical home (PCMH) model of care according to TJC standards and elements of performance in which patients are engaged in a direct relationship with a chosen provider who coordinates a cooperative team of healthcare professionals, takes collective responsibility for the comprehensive integrated care provided to the patient, and advocates and arranges appropriate care with other qualified providers and community resources as needed.
  • Provider must document and code all patient encounters in CHCS/AHLTA.

Physician - Family Practice Job Qualifications:

  • 5 years experience
  • BC/BE
  • DEA
  • MD or DO
  • Graduate from an accredited medical school in the United States or Canada.*
  • Basic Life Support
  • Licensure: active, current, unrestricted. Any U.S. state.
  • Computer competency including at least 30 WPM
  • Internship/Residency: FP - approved by the Accreditation Council for Graduate Medical Education or the Committee on Postdoctoral Training of the American Osteopathic Association.

*This degree must have been accredited by the Council on Medical Education of the AMA; AAMCs; Liaison Committee on Medical Education; Commission on Osteopathic College Accreditation of the American Osteopathic Association, or an accrediting body recognized by the U.S. DOE and ACGME at the time the degree was obtained (e.g. RCPSC), College of Family Physicians of Canada (CFPC).
