Community Support Specialist - Berryhill Center

FTE: 1.0 (Full-time, 40 hours per week)Shift: M-F 8a-4:30p

Responsibilities

* Provide direct care to patients to maintain successfully and safely in independent livi

* Provides at home or office, in community or by phone visits to assigned patients to monitor home conditions and supervise/support activities of daily livi

* Completes Functional Assessment and Initial Treatment Plan within 30 days of admission to program, in conjunction with patient.

* Completes review of Functional Assessment and Treatment Plan every 6 months, in conjunction with patient.

* Provides education on medication and symptom management to assigned pati

* Assists patients in determining and obtaining needed If multiple service involvement, coordinates services on behalf of the patient.

* Conducts ongoing assessment of assigned patients' mental health.

* Provides transportation, when needed, for medical appointments and other client related needs.

* Documents service provision, as required by standards, in a timely

* Assists with appointment scheduling and message taking regarding symptoms and medications as

* Maintains strict patient confidentiality/follows HIPAA guideli

* Participates in Center's operations demonstrating professional behaviors to include supporting goals of the Center and maintain awareness of resource utilization.

* Provides clinical support to physician/ARNP/ PA for complex cases requiring case management, for example, contacting doctors and other health care providers to coordinate services.

* Works cooperatively with referral agencies in coordinating services to ensure optimal consistency of service provisions.

* Maintains and develops professional relationships with all outside agencies and external customers by communicating in an efficient, professional and courteous

* Demonstrates positive customer

* Maintains compliance with Human Resources policies and procedures including the dress code.

Qualifications

Education:

* Preferred Associates or Bachelors degree in Social Work, Human Services, or another Human Service related field.

Licensure:

* None

Experience:

* One (1) to two (2) years preferred experience within a behavioral health setting.

Skills/Abilities:

* Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively.

* Must demonstrate knowledge and skills required for specialized area of assignment.

* Must demonstrate knowledge and skills necessary to provide care appropriate to adult and geriatric patients.

* Must demonstrate effective communicationskills, i.e., demonstrates effective written and oral communication skills for use with physicians, co-workers, other clinic/hospital personnel, as well as the public.

* Requires good organizational skills.

* Must be able to objectively perform initial assessment of patient's condition along with needs related to adult and geriatric behavioral health patient.

* Must possess the ability to effect planned change in apositive manner.

* Ability to work effectively with others, think creatively and communicate effectively,

* Requires working knowledge of community agencies. Utilizes computer documentation effectively.

* Must have a valid driver's license, auto insurance and reliable transportation.
