Transportation Driver

Responsibilities:

Transport residents to/from medical appointments in a safe, timely and efficient manner, according to the established policies, procedures and objectives of Grace Lutheran Foundation/ Heritage of Elmwood (the "organization"). Upholds the Mission of Grace Lutheran Foundation/ Heritage of Elmwood.

Special Features/Requirements:

* Must hold valid Wisconsin Drivers License as described by Wisconsin Department of Transportation.

* Must provide evidence of insurability and meet Foundation's auto insurance carrier's requirements.

* Basic knowledge of vehicle operation.

* Proof must be furnished of current American Red Cross basic first aid and CPR certification.

* Must have excellent communication skills, preferably previous experience working with elderly.

* Follows/adheres to organization, federal/state regulations and guidelines, including resident/client rights; maintains, neat, well groomed appearance, adhering to dress code of the organization.

Essential Functions:

* Transfers residents in/out of vehicle and accompanies residents to designated appointment area in a courteous manner.

* If necessary, will remain with resident in reception area.

* Adheres to the resident appointment time schedule.

* Maintains a daily transportation log and submits documentation to Resident Transportation Coordinator.

* Reports any vehicle deficiencies or repairs needed to Resident Transportation Coordinator. Deficiencies must be corrected before transporting residents.

* Maintains open communication.

* Has access to confidential information/records and maintains confidentiality. Nature of medical appointments must not be discussed with anyone other than authorized representatives of the facilities directly involved.

* Follows established rules and regulations for vehicle operation established by Wisconsin Department of Transportation.

* Projects a positive image of the organization in dealing with residents/clients, coworkers, public.

Physical Requirements:

* Pushes wheelchair-bound residents

* Loads and unloads wheelchairs from the van

* Operates transport vehicle lift equipment

* Frequently reaches at, above or below shoulder height

* Frequent bending, stooping, squatting, sitting and standing

* Frequently walks on uneven ground; occasionally kneels

* 60% of the work day is spent sitting, 20% standing, 20% walking.

* 80% of the day is spent outdoors, 20% indoors.

* Must be able to communicate orally and/or in writing with residents, coworkers, families, public.
