About UFW Foundation: For 17 years, the UFW Foundation has mobilized farm workers and their organizations across the country to advocate for more equitable policies, such as immigration reform, pesticide protections, heat standards, hazard pay and other worker protections. We engage constituents in systemic change to break the cycle of poverty while also providing critical services. In fact, the UFW Foundation is the largest federally accredited immigration legal service provider in the state of California. The organization has staff in California, Arizona, Georgia, Illinois, and Michigan.
SUMMARY: The Case Manager will work with UFW Foundation clients and their families to identify needs and gaps in services, assist families in accessing services related to housing, transportation, health, food, education, mental health, and child care. The Case Manager will also be expected to provide leadership in establishing and sustaining relationships with local and state social services partners and any other appropriate disaster recovery assistance needed by families impacted by storms.
ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING: (This is not an exhaustive list)
- Aid community members with intake and application for the Storm Assistance for Immigrants program.
- Aid community members with referrals for housing, food, medical, legal, mental health and other community resources related to basic needs, and follow up as necessary.
- Assist staff in working with families in crisis and transition.
- Complete referrals to services, assist families in completing intake requirements for these services,
- Develop and maintain a knowledge base of social services available to clients in each service area.
- Develop relationships with community coalitions in areas of high client concentration.
- Establish working relationships and partnerships with local service providers and administrators in each service area to enhance the referral process.
- Evaluate client needs (screen and assess for social services’ needs).
- Maintain confidentiality and security of case information.
- Maintain documentation regarding attempts to access services and service delivery.
- Provide case management using organizational database or other assigned platforms.
- Track and report client data, outreach actions and outcomes, and general information for program evaluation, performance quality, and development purposes.
- Other tasks as needed.
- May involve the coordination and supervision of volunteers/interns.

COVID-19 Vaccination Requirement – The UFW Foundation requires staff, volunteers, and independent contractors to be immunized against COVID-19 or declare a medical or religious exemption. First booster shots are also required. Any candidates advanced in a currently open search process should be prepared to comply with this requirement as a condition of employment upon hire.
EDUCATION and/or EXPERIENCE:
Associate's degree or equivalent from two-year college or technical school; and two years or more of related experience and/or training; or equivalent combination of education and experience.

- Valid California Driver’s License.
- Valid auto insurance
