General Responsibilities

Responsible for building, management and performance of the Post-Acute network of preferred providers. This position is responsible for all aspects of provider contracting, network development and steerage.

Coordinates and implements actions to develop and maintain Continuing Care Network (CCN) for affiliated/unaffiliated skilled nursing facilities (SNFs) long-term care hospital (LTACH) home health agencies (HHAs) and hospice and inpatient rehabilitation facility (IRF). Manages and coordinates integration of unaffiliated and affiliated post-acute providers with the goal of an acute/post-acute seamless care continuum for Mercy Accountable Care.

Responsible for driving and supporting post-acute care coordination programs, with focus on quality of outcomes and superior service. Promotes a collaborative, open, and inclusive work environment within a highly matrixed organization. This position serves as one of the primary advisor to the organization's senior management on matters related to post acute care coordination programs. Monitors and oversees compliance of customer service, stakeholder relations, and financial goals and objectives as developed by the senior management.
