Provides behavioral health and care management services to diverse populations in centralized, inpatient and ambulatory care settings. Prefer bi-lingual (Spanish) and experience with Pediatric patients.

Responsibilities include:

  • Completes assessment of psychosocial needs and provides short-term therapeutic counseling for high risk, complex patients and their families.
  • Coordinates care, identifies resources, and facilities transitions of care.
  • Develops collaborative relationships with providers and care team members to facilitate high quality, low cost care.

Accountable for consistently achieving productivity and quality metrics.

Education and Experience:

Master's in Social Work.

LCSW in North Carolina

1-2 years of clinical or behavioral health experience in a health care setting caring for patients with complex medical conditions.

Experience in case management, utilization review or quality improvement desirable, Pediatric patient population.

BLS preferred.
