Exercise Physiologist

Main Function: Assesses nursing and learning needs of patients with cardiac diagnoses in both inpatient and outpatient settings. Develops teaching and exercise plans for patients in accordance with GXT and/or 220-age and physician recommendations. Functions in a comprehensive health team fostering a healthy lifestyle of patients and staff.

Required Job Specific Competencies:

* Adheres to the standards of practice established by the Cardiac Rehabilitation Department providing nursing care based on Nursing Process. Demonstrates judgment and decision making skills needed to meet the nursing needs of the cardiac rehabilitation patient. Effectively communicates through technical, written and verbal communication Means.

* Provides for the identified health education and other learning needs of the patient, family or significant other, new orientees, and hospital ancillary personnel.

* Demonstrates knowledge of the principles of growth and development over the life span.

* Possesses the ability to assess data reflective of the patient's status and interpret each patient's requirements relative to age-specific needs.

* Anticipates patient needs and outcomes of treatment. Plans ahead. Seeks clarification of any orders that are not understandable or appropriate. Confers with admitting physician or medical director regarding issues that may cause problems with planned exercise program. Documents all care, observations, response to treatment and patient feedback utilizing all specialized documentation records per policy.

* Adapts a teaching plan of care that addresses those needs identified by the Rehabilitation Team. Once the plan is developed, communicates it to the patient to enlist their input and cooperation for meeting goals. With input from the patient and significant other, individualizes the standard Cardiac Rehabilitation teaching material to meet their learning needs. Gives pre-exercise instructions to patient and significant others as appropriate.

* Utilizes audiovisual material and answers questions related to the information presented. Assures that the audiovisual material is current.

* Participates in patient care conferences or department sponsored inservices on a regular basis. Acts as facilitator for outpatient education group, Mended Hearts in the absence of the department Supervisor. Promotes behavioral changes consistent with medication compliance, smoking withdrawal, dietary modifications and appropriate exercise habits.

* Operates all exercise and monitoring equipment. Accurately computes patient exercise prescription from GXT and verifies target heart range with cardiologist or medical director.

* Reports any change in patient condition immediately to the physician supervising the Cardiac Rehabilitation Program. Plans for and supervises emergency drills and Mock Code Blue drills on a regular basis. Gives appropriate feedback and constructive criticism to staff.

* Demonstrates accurate cardiovascular and pulmonary physical assessment including inspection, palpitation, percussion and auscultation. Identifies dysrhythmia and correlates appropriate interventions. Interprets twelve lead electrocardiograms accurately with particular emphasis on identification of ischemic changes. Correlates appropriate interventions based on interpretations. Performs emergency cardiac care measures, including defibrillation, initiation and maintenance of intravenous fluids, giving emergency medication and administering cardiopulmonary resuscitation and/or advanced cardiac life support measures.

* Provides appropriate and quality care based on the results of current nursing research and quality assurance monitoring results. Maintains adequate level of all supplies, equipment and medications to provide timely care to every patient. Accurately documents Cardiac Rehabilitation logbook and charge information per policy. Is knowledgeable about the safe use, maintenance and cleaning of all specialized equipment in the department. Checks emergency supplies and equipment for proper function per unit policy.

* Participates in the unit base Quality Assurance program. Incorporates quality assurance findings into the plan of patient care. Incorporates quality assurance findings into practicing of unit protocols.

* Actively guards the confidentiality of sensitive information including but not limited to patients and staff of the health system.

* Actively supports hospital goals and imperatives and lives the CARES values at all times.

RequirementsQualifications: Required American Heart Association BLS and ACLS certification current. BSN preferred or required within five years of employment.

Education: Graduate of an approved school of nursing or, having met requirements by the NYS Education Department, is eligible for licensure, or currently holds a NYS license as a registered nurse. Preferred: Completion of a comprehensive Cardiac Rehabilitation course and a basic arrhythmia course.

Experience: Broad base of clinical experience in the hospital setting, including a minimum of 24 months in cardiac care preferred.

Complexity of Duties: Uses nursing/professional judgment, within the scope of practice, to assess, plan, implement and evaluate individual customer plan of care. Provides education and care for patients, including independent emergency care in unplanned events. Demonstrates effective use of computerized documentation tools.
